| BIRTH CERTIFICATES | |
| Birth certificates are mandatory for new registrants. This year adults must be registered at the same time as the youth players due to registration requirements enacted by the Ontario Soccer Association. | |
| MAILING YOUR REGISTRATION | |
| If
you are mailing your registration and payment to the club, please
ensure that you complete and sign the registration form, for the form
is very extensive. If you are a "new registrant" with our club, please ensure to send a copy of your birth certificate, the copy of your birth certificate will be shredded. Mailing Address: Caledon Soccer Club, PO Box 119 Stn Main Caledon East, Ontario L7C 3L8 You may drop off your registration form and payment through the mail slot at the Club office, 15426 Airport Rd, (at Old Base Line Rd) Caledon East. |

| PLAYER PLACEMENTS | |
| Attention
Players and Parents: The Club will not entertain requests for player placements. The Club has to ensure that teams are properly balanced to make sure it is fun for all who participate. Thank you for your cooperation. |

| LATE FEE | |
|
Youth: A late fee of $25.00 applies after March 7, 2009 Registration fee per player is $205.00 Adult: A late fee of $25.00 applies after March 7, 2009 Registration fee per player is $250.00 A late fee of $50.00 applies after May 26, 2009 Registration fee per player is $275.00 |

| REFUNDS | |
|
All refund requests shall be made in writing and properly
documented. Refunds or credits may be requested only for serious injury or major medical issues confirmed by a doctor’s certificate. An administration fee of $50.00 per player applies to refund requests made after March 7, 2009. No refunds will be made after May 1st, 2009. |
