Youth: $180.00 per player
Adult: $200.00 per player until March 24th.
$225.00 after March 24th.

Please make sure that pages 1, 2 & 5 are completed in full, signed by the appropriate person & returned with pages 3 & 4.
Youth - Adult |
| BIRTH CERTIFICATES |
| |
Birth certificates are mandatory for new registrants. This year adults must be registered at the same time as the youth players due to registration requirements enacted by the Ontario Soccer Association. |
| MAILING YOUR REGISTRATION |

If you are mailing your registration and payment to the club, please ensure that you complete and sign the registration form, for the form is very extensive.
If you are a "new registrant" with our club, please ensure to send a copy of your birth certificate, the copy of your birth certificate will be shredded.

| PLAYER PLACEMENTS |

Attention Players and Parents:
The Club will not entertain requests for player placements. The Club has to ensure that teams are properly balanced to make sure it is fun for all who participate.

| LATE FEE |
| |
Youth:
A late fee of $25.00 applies after March 24th, 2010
Registration fee per player is $205.00
Adult:
No late fee, but after May 26, 2010 there is no guarantee that you will get on a team as they will already have been organized.

| REFUNDS |
| |
All refund requests shall be made in writing and properly documented.
Refunds or credits may be requested only for serious injury or major medical issues confirmed by a doctor’s certificate.
An administration fee of $50.00 per player applies to refund requests made after March 24, 2010
No refunds will be made after May 1st, 2010 for Youth, May 15th for Adult Programs |
